Scoring & Output

  • Each domain = average of items (e.g., HQC = mean of 3 questions).

  • Overall “Co-Care Health Score” = average of all domains (out of 5, or converted to a 100-point index).

  • Provide leaders with both:

    • Overall Score → snapshot of Co-Care Workplace Wellbeing.

    • Domain Scores → show where they are strongest and where growth is needed.


Co-Care Scoring Guide

Each pillar is scored 1–5. Domain averages can be combined into an Overall Co-Care Score.

0 – 2.5 = Needs Care

⚠️ The environment is running low on Co-Care. Signs of strain may include limited collaboration, weak connections, or a lack of optimism. Leaders should focus on rebuilding trust, creating space for input, and sparking positive emotions to refresh energy across the team.

2.6 – 3.5 = Growing

🌿 The environment shows healthy signs of Co-Care but still has room to expand. Collaboration, resilience, and positivity are emerging, though not yet consistent. Leaders here can accelerate progress by amplifying strengths, fostering high-quality connections, and leaning into opportunities thinking.

3.6 – 5.0 = Thriving

✨ The environment is rich in Co-Care. Collaboration is strong, trust and connection are high, and positive emotions are fueling resilience and innovation. Leaders should celebrate this strength, sustain momentum, and broaden and build practices that keep the team aligned and energized for the future.
